Records Release

Obtaining copies of your medical records is one of your rights as a patient.

You may request your medical records by:

1. Visiting our office to fill out a Medical Record Release Form.

2. Download the Medical Record Release Form, Fill it out, and fax it directly to us at (307) 745‑8149 OR email it

3. Access your Athena Patient Portal to obtain DTC visit notes.

Please remember to bring a government-issued photo ID. If you do not provide proper identification and authorization, we are not able to release your records.
